  • Prostate cancer that has progressed/spread to other parts of the body and still responds to a medical or surgical treatment that scales down the testosterone. This is known as metastatic castration-sensitive prostate cancer (mCSPC).
  • Prostate cancer that has not progressed/spread to other organs/parts of the body and no longer responds to a medical or surgical treatment that scales down the testosterone. This is known as non-metastatic castration-resistant prostate cancer (nmCRPC).

Dosage

The recommended dose of Apnat (Apalutamide) is 240 mg (four 60 mg tablets) administered orally once daily with or without food. Swallow the tablets whole. Patients should not stop their treatment without talking with their health specialist first.

Patients should start/continue a GnRH (gonadotropin-releasing hormone) analog therapy during their treatment of Apalutamide unless they had surgery to scale down the level of testosterone in the blood (surgical castration).

In case of a missed dose, take a normal dose of apalutamide 60 mg as promptly as possible on the same day. Return to normal dose schedule on the following day. Do not take any extra tablets to make up for the missed dose. In case of an overdose, call your health specialist immediately.

Side effects

The most commonly reported apalutamide side effects are decreased appetite, diarrhea, fatigue, falls, arthralgia, rash, weight decreased, hypertension, hot flush, and fracture.

Warnings And Precautions
  • Ischemic cardiovascular events, including events that lead to death, may occur in those taking treatment with apnat 60 mg. Monitor patients for signs/symptoms of ischemic heart disease. Optimize management of cardiovascular risk factors, such as dyslipidemia hypertension, or diabetes. Discontinue treatment for grade 3 & 4 events.
  • Fractures may occur in those receiving treatment with apalutamide. Assess respective patients for fracture risk. Evaluate and manage patients at risk for fractures as per the standard treatment guidelines and consider the use of bone-targeted agents.
  • Falls may occur in those receiving Apnat (apalutamide tablets 60 mg) treatment with increased frequency in the elderly. Evaluate respective patients for fall risk. In a randomized study, falls were not related to loss of consciousness or seizure.
  • A seizure may occur in patients receiving treatment with Apnat. Permanently discontinue Apnat in those who develop a seizure during treatment. Do not engage in any activity where sudden loss of consciousness could cause harm to themselves or others.
  • Apalutamide`s mechanism of action suggests that this medicine can cause fetal harm and loss of pregnancy if administered to a pregnant female. Males with a female partner of reproductive age should use appropriate contraceptive measures during treatment and 3 months after the final dose of apnat tablets 60 mg.
  • Apalutamide pills may be responsible for causing fertility complications in males. Avoid donating sperm during treatment with this medicinal product and for the 3 months after the final dose of Apalutamide.
Storage and Handling

Store Apnat at temperature 20°C to 25°C (68°F to 77°F); excursions permitted to 15°C to 30°C (59°F to 86°F). Store tablets in the original package. Do not discard desiccant. Protect this medication from light as well as moisture.

Drug Interactions of Apnat

Concomitant use with medications that are sensitive substrates of CYP2C8/CYP3A4, CYP2C19, CYP2C9, UGT, P-gp, BCRP, or OATP1B1 may be responsible for causing loss of activity of these medicines.
