What is Hepatitis-B Immunoglobulin, and what it is used for?

Hepatitis-B Immunoglobulin (HBIG) is a human immunoglobulin that is used:

  • to prevent hepatitis B infection following exposure to the hepatitis B virus (post-exposure prophylaxis)
  • to prevent hepatitis B recurrence following liver transplantation in patients with hepatitis B.

Usual dose (post-exposure prophylaxis): Each neonatal hepatitis b immunoglobulin dose is 0.5 mL, administered once intramuscularly. An adult dose of Hepatitis-B Immunoglobulin is 0.06 mL/kg, administered once intramuscularly.
Usual dose (following liver transplant): Each dose of 35 mL (10,920 international units) Hepatitis-B Immunoglobulin will be administered by an intravenous injection taking around 20 minutes.

The typical dose schedule is as follows:

  • first dose during liver transplantation operation
  • daily doses for the 1’st week post-operative
  • once every 2 weeks for the first 3 months post-operative
  • once a month thereafter
Side Effects

The most commonly reported side effects due to hepatitis b immunoglobulin include: chills, headaches, fever, vomiting, nausea, allergic reactions, arthralgia (pain in joints) and moderate low back pain. These side effects are generally mild, but in case they require treatment ask your health specialist.

Warnings and Precautions
  • Immune globulins like Hepatitis-B Immunoglobulin may impair the effectiveness of some live virus vaccines such as rubella (German measles), measles, mumps and varicella (chicken pox). Discuss with your health specialist in case you have been recently vaccinated.
  • Do not use hepatitis b immunoglobulin medicine in patients with a previous or known history of allergic reactions to blood products and in patients deficient in IgA, a specific kind of blood protein.
  • Liver transplant patients need to be monitored on a regular basis for serum anti-HBs antibody levels.
  • Anaphylactic or allergic reactions are rare. These reactions may appear in patients with a known history of allergy to blood products or in patients lacking the IgA blood protein.
  • The hep b immunoglobulin is made from human plasma which may have causative agents of viral diseases. Probability of getting a disease from this medicinal product has been decreased by screening plasma donors, testing for the existence of viruses and by inactivating and eliminating some viruses during manufacturing. Although, there is still a probability that plasma products could transmit disease.
Supplied/Storage and handling:

Store vials of hep b immunoglobulin under refrigeration (2°C to 8°C). Do not freeze. The medicinal product should be brought to body or room temperature prior to use. The solution should be slightly opalescent or clear. Avoid using solutions that are cloudy or have deposits.
