What is Midostaurin, and what it is used for?

Midostaurin is a kinase inhibitor used for the treatment of adult patients with:

  • Newly diagnosed AML or acute myeloid leukemia that is FLT3 mutation +Ve, together with standard cytarabine and cytarabine consolidation and daunorubicin induction.
  • Aggressive systemic mastocytosis (ASM), mast cell leukemia (MCL) or systemic mastocytosis with associated hematological neoplasm (SM-AHN).

Dosage

The recommended dose of midostaurin capsule for patients with acute myeloid leukemia is 50 mg orally twice daily with food on Days 8’th to 21’st of each cycle of induction with cytarabine and daunorubicin and on Days 8’th to 21’st of each cycle of consolidation with high-dose cytarabine.

The recommended Midostaurin dose for patients with ASM, MCL & SM-AHN is 100 mg orally twice a day with food. Continue treatment until the disease progresses or unacceptable toxicity occurs.

Side Effects

The most commonly reported side effects m due to midostaurin capsule 25 mg include:

  • AML: febrile neutropenia, mucositis, nausea, vomiting, headache, musculoskeletal pain, petechiae, epistaxis, device-related infection, upper respiratory tract infection, and hyperglycemia.
  • ASM, MCL or SM-AHN: constipation, musculoskeletal pain, vomiting, diarrhea, edema, abdominal pain, fatigue, upper respiratory tract infection, pyrexia, headache, dyspnea, and nausea.
Warnings and Precautions
  • Assess patients for toxicity at least weekly for the initial 4 weeks, every other week for the next 8 weeks, and monthly thereafter during treatment.
  • Cases of interstitial lung disease and pneumonitis, some fatal, have appeared in patients treated with midostaurin 25 mg as monotherapy or with chemotherapy. Evaluate patients for pulmonary signs and symptoms.
  • Discontinue treatment with midostaurin in those patients who experience signs or symptoms of interstitial lung disease (ILD) or pneumonitis without an infectious etiology.
  • Treatment with midostaurin can be responsible for causing vomiting, nausea, and diarrhea. All the patients should contact their health specialist if these symptoms appear or are persisting despite supportive medicines.
  • The midostaurin capsule 25 mg may cause fetal harm if administered to a pregnant woman. All the respective females are advised to inform their healthcare provider promptly of a known or suspected pregnancy.
How Supplied/Storage and Handling:

Store the capsules at 25°C (77°F); excursions permitted to 15°C to 30°C (59°F to 86°F). Store in the original package in order to protect from moisture.
