What is Nimodipine, and what it is used for:

Nimodipine is used for the prevention of ischaemic neurological deficits following aneurysmal subarachnoid haemorrhage.

Dosage

The recommended dose of Nimodipine is two tablets of 30 mg (nimodipine 60 mg) at 4-hourly intervals (total daily dose 360 mg) to be taken with water. Swallow the tablets whole with a little liquid, with or without food. Avoid taking grapefruit juice. Prophylactic administration should commence within 4-days of onset of subarachnoid haemorrhage and should be continued for 21 days.

In an event of surgical intervention, administration of tablets should be continued to complete the 21-days treatment period.

In patients who develop adverse reactions the dose of Nimodipine should be decreased as necessary or the treatment discontinued.

  • Solution for Infusion: Nimodipine is also supplied as 0.02 % clear yellow sterile solution for intravenous use. For the initial couple of hours of treatment 1 mg of nimodipine, i.e. 5 ml Nimodipine solution, (about 15 µg/kg bw/h), should be infused each hour through a central catheter. In case it is well tolerated, the dose of Nimodipine should be increased after 2-hours to 2 mg nimodipine, i.e. 10 ml solution/hour (about 30 µg/kg bw/h), providing no severe decrease in blood pressure is observed.
    Patients of body weight below 70 kg or with unstable blood pressure need to be started on a dose of 0.5 mg nimodipine per hour (2.5 ml solution), or less if necessary.
  • Oral Solution: Nimodipine is also supplied as Oral Solution in the dosage strength of 60 mg per 20 mL (3 mg/mL), pale yellow solution. The standard oral dosage is 20mL (60 mg) every 4-hours for 21-consecutive days. This medicinal product should be administered only enterally (e.g., oral, nasogastric tube, or gastric tube route). Avoid administering intravenously or by any other parenteral routes. For all the routes of administration, begin therapy within 96 hours of the onset of SAH. Administer 1-hour before a meal or 2-hours after taking a meal for all the routes of administration.
Side effects

The most commonly reported Nimodipine side effects include: increased heart rate (tachycardia), decreased blood pressure, headache, nausea, stomach discomfort, muscle cramp, rash, and edema (swelling over the whole body).

Warnings And Precautions
  • The nimodipine tablets must not be used in case of hypersensitivity to the active substance or to any of the excipients in this medicinal product.
  • Nimodipine should not be used in patients during or within 1 month of a myocardial infarction or an episode of unstable angina.
  • The nimodipine uses are restricted if used along with rifampicin or the antiepileptic drugs, phenobarbital, phenytoin or carbamazepine, as the efficacy could be significantly reduced when concomitantly used.
  • The nimodipine tablet should be used with care when cerebral oedema or severely raised intracranial pressure exist.
  • Reduced drug clearance may develop in cirrhotic patients receiving nimodipine and, therefore, close monitoring of blood pressure is required in these patients.
  • The tab nimodipine should not be administered concomitantly with Nimodipine solution.
  • Respective nursing mothers are advised not to conceive pregnancy and not to breast-feed when taking this medicine.
How Supplied/Storage and Handling:

Store nimodipine 30 mg tablets in the original foil package at 25°C (77°F), excursions permitted to 15°C-30°C (59-86°F), away from moisture, heat, and light. Tablets should be protected from the light and freezing.
