What is Nirmatrelvir Ritonavir, and what it is used for?

The FDA has issued an EUA for the emergency use of the unapproved and co-packaged medications Nirmatrelvir, a SARS-CoV-2 main protease inhibitor, and Ritonavir, an HIV-1 protease inhibitor and CYP3A inhibitor, for the treatment of mild-to-moderate Coronavirus Disease 2019 (COVID-19) in adults and pediatric patients (aged 12 years and older weighing at least 40 kg) with positive results of direct SARS-CoV-2 viral testing, and who are at higher risk for progression to severe COVID-19, including hospitalization or death.


Nirmatrelvir tablets co-packaged with ritonavir tablets, and must be co-administered. The proposed dosage is 300 mg nirmatrelvir (2 × 150 mg tablets) with 100 mg ritonavir (1 × 100 mg tablet) with all three tablets taken together by mouth (orally) twice daily for 5 days. Both nirmatrelvir and ritonavir tablets can be taken with or without food. Swallow the tablets whole and do not chew, break, or crush them.

The 5-day treatment course of Nirmatrelvir/Ritonavir needs to be initiated as promptly as possible once the diagnosis of COVID-19 has been made, and within 5 days of signs/symptom onset. Should a patient require hospitalization due to severe or critical COVID-19 after initiating treatment with

Nirmatrelvir/Ritonavir, the patient should finish the full 5-day treatment course per the health specialist’s discretion.
If the patient misses any dose of Nirmatrelvir/Ritonavir within 8 hours of the time-span it is usually taken, the patient should take it as quickly as possible and resume the general dosing schedule. If the patient misses any dose by excess than 8 hours, the patient should not take the missed dose and instead take the next dose at the regularly scheduled time. Do not take a couple of doses in order to make up for a missed dose.


Side effects

The most commonly reported Nirmatrelvir Ritonavir side effects include: dysgeusia, hypertension, diarrhea, and myalgia.

Warnings And Precautions
  • Use of Nirmatrelvir/Ritonavir is restricted in patients with a history of clinically significant hypersensitivity reactions (e.g., toxic epidermal necrolysis (TEN) or Stevens-Johnson syndrome) to nirmatrelvir/ritonavir or any other components of the product.
  • Nirmatrelvir must be co-administered along with ritonavir. Failure to correctly co-administer nirmatrelvir ritonavir tablets may result in plasma levels of nirmatrelvir that are not sufficient to achieve the required therapeutic effect.
  • Finishing the full 5-day treatment course and continued isolation in accordance with public health recommendations are crucial to maximize viral clearance and minimize the transmission of SARS-CoV-2.
  • Hepatic transaminase elevations, jaundice, and clinical hepatitis have been noted in patients receiving ritonavir. Therefore, caution should be exercised when using Nirmatrelvir/Ritonavir to patients with pre-existing liver diseases, liver enzyme abnormalities, or hepatitis.
  • Because nirmatrelvir tablets are co-administered with ritonavir, there may be a probability of HIV-1 establishing resistance to HIV protease inhibitors in people with uncontrolled or undiagnosed HIV-1 infection.
Storage and Handling

Store Nirmatrelvir 150 mg and Ritonavir 100 mg Tablets at USP controlled room temperature 20°C to 25°C (68°F to 77°F); excursions permitted between 15°C to 30°C (59°F to 86°F).
