What is Pentoxifylline, and what it is used for?

Pentoxifylline is used for the treatment of patients with intermittent claudication on the basis of chronic occlusive arterial disease of the limbs. Pentoxifylline can ameliorate the function and signs/symptoms but is not evaluated to replace the more definitive therapy, such as surgical bypass, or elimination of arterial obstructions during the treatment of peripheral vascular or peripheral artery disease.


The usual dosage of Pentoxifylline tablets is one tablet (400 mg) three times daily with meals. While the effect of Pentoxifylline may be seen within 2-4 weeks, it is recommended that treatment be continued for at least 8 weeks.

Digestive and central nervous system adverse reactions are dose related. If patients develop these effects it is recommended that the dosage be reduced to one tablet twice daily (800 mg/day). If adverse reactions persist at this reduced dosage, the administration of Pentoxifylline should be discontinued.

Side effects

The most commonly reported side effects due to pentoxifylline 400 mg include: stomach discomfort or upset, bloating, nausea, vomiting, indigestion, flushing, and dizziness. Uncommon and rare side effects include rash, angina, palpitations, hypersensitivity, itchiness, hives, bleeding, arrhythmias, hallucinations, and aseptic meningitis.

Warnings And Precautions
  • The pentoxifylline er 400 mg should not be used in patients with recent cerebral or retinal hemorrhage or in patients who have previously exhibited intolerance to this medicinal product or methylxanthines such as theophylline, caffeine, and theobromine.
  • Patients on Warfarin must have more frequent monitoring of prothrombin times, while patients with other risk factors complicated by hemorrhage should have periodic assessments for bleeding including, hematocrit or hemoglobin.
  • At the initial sign and symptoms of anaphylactic or anaphylactoid reactions, administration of pentoxifylline er 400 mg tab should be interrupted or discontinued.
  • Concomitant use of Pentoxifylline and theophylline-containing medicines leads to increased theophylline levels and theophylline toxicity in some individuals. Such patients should be precisely evaluated for signs of toxicity and have their theophylline dosage adjusted as necessary.
  • Pentoxifylline generic medication has been used concurrently with antihypertensive medicines, beta blockers, diuretics, digitalis, and antiarrhythmics, without observed problems. A decrease in blood pressure has been observed in patients treated with Pentoxifylline; monitor periodic systemic blood pressure in patients with concomitant antihypertensive therapy.
  • Postmarketing cases of enhanced anticoagulant activity have been observed in patients concomitantly taking treatment with vitamin K antagonists and pentoxifylline er. Monitor anticoagulant activity in these patients when pentoxifylline is introduced or the dose is changed.
  • Pentoxifylline and its metabolites are known to be excreted in human milk. Due to the potential for tumorigenicity shown for pentoxifylline in animals, a decision needs to be made whether to discontinue nursing or discontinue the medicine, taking into account the importance of the medicine to the mother.
