What is Sacubitril Valsartan, and what it is used for?

Sacubitril Valsartan is a fixed dose combination medicine comprising sacubitril, a neprilysin inhibitor, and valsartan, an angiotensin II receptor blocker, used to reduce the risk or probability of cardiovascular death and hospitalization for heart failure in individuals with chronic heart failure (NYHA Class II-IV) and reduced ejection fraction.
This medicine is usually administered in conjunction with other heart failure therapies, in place of an ACE inhibitor or other ARB.

Dosage

The recommended starting dose of sacubitril / valsartan is 49/51 mg orally twice-daily. Double the dose of this medicine after 2 weeks to 4 weeks to the target maintenance dose of 97/103 mg twice daily, as tolerated by the patient.

Reduce the starting dose of sacubitril/valsartan to 24/26 mg twice a day for those patients who are not currently taking an angiotensin-converting enzyme inhibitor (ACEi) or an angiotensin II receptor blocker (ARB) or previously taking a low dose of these agents, patients with severe renal impairment, and patients with moderate hepatic impairment. Double the sacubitril/valsartan dose every two to four weeks to the target maintenance dose of 97/103 mg twice a day, as tolerated by the patient.

Side effects

The most commonly reported side effects due to Sacubitril Valsartan include: hyperkalemia, renal failure, hypotension, cough, and dizziness.

Warnings and Precautions
  • The sacubitril valsartan uses can be responsible for causing fetal harm if administered to a pregnant woman. Use of drugs that act on the renin-angiotensin system during the 2’nd and 3’rd trimesters of pregnancy reduces fetal renal function and increases fetal and neonatal morbidity and death.
  • Concomitant use of Sacubitril/Valsartan and an angiotensin-converting enzyme (ACE) inhibitor is contraindicated. Switching from an ACE inhibitor to Sacubitril/Valsartan allows a washout period of 36 hours between administration of the two medicines.
  • The sacubitril valsartan generic may cause angioedema. In case angioedema occurs, interrupt this medication immediately, provide apt therapy, and assess for airway compromise.
  • Patients with a previous history of angioedema may be at higher risk of angioedema with drug sacubitril valsartan. It must not be used in patients with a previous history of angioedema related to previous ACE inhibitors or ARB therapy. It should not be used in patients with hereditary angioedema.
  • The sacubitril valsartan tablets lowers the blood pressure and may be responsible for causing symptomatic hypotension. Patients with an activated renin-angiotensin system, such as volume- or salt-depleted patients (e.g., those being treated with high doses of diuretics), are at higher risk.
  • Closely assess serum creatinine, and down-titrate or interrupt sacubitril and valsartan therapy in patients who develop a clinically significant reduction in renal function.
  • As with all medicines that affect the RAAS, sacubitril and valsartan tablets may increase blood urea and levels of serum creatinine in patients with bilateral or unilateral renal artery stenosis. In patients with renal artery stenosis, assess renal function.
  • Through its actions on the RAAS, hyperkalemia may appear with this medicine. Evaluate serum potassium periodically and treat appropriately, mainly in patients with risk factors for hyperkalemia such as diabetes, severe renal impairment, hypoaldosteronism, or a high potassium diet. Dosage interruption or reduction of sacubitril valsartan may be needed.

Store the tablets at 20°C-25°C (68°F to 77°F), excursions permitted between 15°C and 30°C (59°F and 86°F). Protect from moisture.
